Please ensure the Cardholder's Name (top of the page), Printed Name (bottom of the page) and Signature (bottom of the page) are all consistent under one person's name. 

 

Once filled in, please return it via email to info@skylinepenthouse.com, including in the body of the email:

1) Full name of the person whose the booking is under

2) Date of the booking

3) Hours (start time and end time) of the booking

4) Name of the studio (it will be "Skyline ______________". For example: "Skyline Loft", "Skyline Boho", "Skyline Penthouse", "Skyline Suite", etc)

 

Please note we must receive and process the refundable security damage deposit at least 48 hours before your booking start time.
